Supportlinks is the Needs Assessment and Service Coordination (NASC) service in the Te Uru Whakamauora (Healthy Ageing and Rehabilitation) directorate which provides access to support services in the Te Pae Hauora o Ruahine o Tararua MidCentral region for people with acute/short term needs, long term conditions, and older people and Kaumatua.

Supportlinks works primarily with a community focus however also has a role within inpatient services supporting the clinical teams with discharge planning, and if needed facilitates access to support for a person when they are discharged.

Are you someone who has?

* An ability to work within a changing health environment
* A mana enhancing and restorative approach within your practice
* A strong desire to support clients and whanau achieve their goals
* An ability to form relationships based on mutual respect, integrity, and credibility
* Effective written, oral and interpersonal skills with an ability to communicate with people from a wide variety of backgrounds and cultures.
* Strong organisational and time management skills
* Skills to use a laptop and learn new assessment software
* A full, current, driver's licence

Supportlinks are looking for a person who is enthusiastic, energetic, resilient, and has a "can do" attitude, to join our supportive team. This permanent full-time position will be based in Palmerston North City and we would welcome applications from candidates who hold a Health or Disability qualification and has significant experience of Needs Assessment Service Coordination Service provision.
